Breast Reconstruction

A large portion of our practice is dedicated to breast reconstruction.  We believe that all women that face breast cancer should be given the option for reconstruction.  We have focused on breast reconstruction for over 10 years and I have seen the results of breast reconstruction continue to improve over this period.  I am humbled at the courage and strength of our patients and find breast reconstruction to be one of the most rewarding aspects of my practice.  We work closely with our general surgery colleagues to design a surgical plan. 

Typically, reconstruction starts at the initial surgery.  Many women elect not to have reconstruction initially but decide later to undergo reconstruction.  We have extensive experience with both types of reconstruction (immediate vs. delayed). 

Breast reconstruction can be broken down in to several categories including tissue expander/implant reconstruction, autologous reconstruction (using your own tissue from another part of your body) or a combination of both.  While we are comfortable with all types of reconstruction, our most common type of reconstruction is tissue expander/implant reconstruction.  An empty implant which can be filled with saline is placed at the time of the mastectomy.  It is placed under the pectoralis muscle.  A material called Alloderm is used to cover the lower portion of the tissue expander.  This is allowed to heal for 3-4 weeks.  The expander is then filled with saline until the patient reaches her desired size.  This is done weekly through a small needle that goes through the skin into a port in the expander.

Once the expander is filled to the desired volume, a second surgery is scheduled to remove the tissue expander and place a gel implant.  This is an outpatient procedure with minimal discomfort and recovery time.

A new nipple is reconstructed at a separate procedure and the areola pigmentation is done after the new nipple is healed.  Revision and touch up procedures can be done at the same time.  I tell my patients that breast reconstruction is a lifelong process and that it is never completed.  As you age, additional surgeries may be indicated.  If only one side is reconstructed, the reconstructed side will age differently than the non-reconstructed side.
